Your First Visit

At Capital Dental, our goal is to provide you and your family with the highest-quality dental care. Because we know that visiting the dentist can be intimidating for some, we also aim to make your appointments as comfortable as possible. We love meeting new patients, so contact us soon to book your first appointment. To ensure that it goes as smoothly as possible, here are some things to expect during your first visit with us.

What to bring

During a patient’s first visit, we always budget a few minutes for paperwork; this step might feel tedious, but it is necessary for us to provide the most thorough care we can. To offer more flexibility and convenience, we have made our New Patient Information form available online. If you wish to review this form and/or complete it ahead of your appointment, please feel free to do so. Simply bring it with you to your appointment.

In addition to the New Patient Information form, we also ask our new patients for a few other pieces of information. These include:

  • Insurance information, including your insurance card
  • The name and date of birth of the person who is the subscriber for the insurance
  • A list of any medications that the patient (either you or a family member) is currently taking

Insurance

For patients covered by a dental insurance plan, we are happy to file the required insurance forms, with a request to have the benefits assigned to our office. Co-payments may be required at the beginning of any planned treatment.

We accept most types of dental insurance, including Aetna, Alwayscare/Starmount, Cigna, Connection Dental, Delta Dental, Dentamax, Guardian, Humana, United Concordia, and United Healthcare. If you don’t see your insurance company listed here, please contact us to check on your coverage.

Payment options

We strive to keep the cost of all of our dental services low. In order to guarantee this low cost, we require payment at the time of service, unless prior arrangements have been made.

For the convenience of our patients, we also accept other forms of payment, including cash, check, and most credit cards. We also take Care Credit, which allows you to make monthly payments for treatments that you need immediately. If you have any questions about payment, please contact us.
